Registered nurse founder of Hope Is At Home

Care You Can Trust, Led by a Nurse Who Cares

Hope Is At Home was founded by a registered nurse who saw firsthand how families struggle to navigate home care — and decided to do something about it.

Our Story

Hope Is At Home was born from a simple but powerful belief: every person deserves to age and heal in the place they love most — their own home. Our founder, a registered nurse with years of clinical experience in hospital and community settings, watched too many families feel lost, overwhelmed, and underserved when trying to arrange home care for their loved ones.

She saw families lost on how to help their loved one stay at home and caregivers working outside their scope of practice. She saw families paying for services they didn't need. She saw seniors declining not because of their condition, but because the right support wasn't in place. So she built something different.

Hope Is At Home is a clinically grounded referral and consultation service built on a foundation of integrity and accountability. Every client engagement begins with a comprehensive in-home evaluation conducted personally by our owner and registered nurse — ensuring that each care plan is informed by direct clinical assessment, not administrative intake. From there, we develop a tailored, evidence-informed plan designed to promote safety, preserve independence, and support long-term wellbeing at home.

Meet Our Founders & RNs

Richard and Elizabeth Moleski, founders of Hope Is At Home

Richard and Elizabeth have a combined 35+ year nursing and medical history. With a background spanning hospital nursing, emergency response, community health, education and home care coordination, our founders bring a rare combination of clinical expertise and genuine compassion to every client they serve.

They established Hope Is At Home because they believed Buffalo families deserved a home care option that was transparent, clinically sound, and truly family-centered. Elizabeth personally conducts every initial evaluation — because she believes the relationship between a nurse and a family should be built on trust from the very first conversation. Richard's background in emergency medicine and clinical oversight helps families feel safe knowing there is someone to call.

Lifelong Buffalo residents, Richard and Elizabeth are deeply rooted in the Western New York community — raising their four children here and staying actively involved in the neighborhoods they serve. Beyond their client work, they are passionate advocates for higher home care standards across the region, and they take pride in helping families understand what it truly takes to age safely and comfortably at home.

Our Philosophy of Care

We believe home care should be honest, appropriate, and centered on the person — not the package.

Dignity First

Every person we serve is treated with the respect and dignity they deserve. We never rush, never dismiss, and never reduce a person to their diagnosis.

Scope-Appropriate Care

Every caregiver on your team works within their professional scope of practice. We never ask anyone to do more than they're qualified and licensed to do — and we never cut corners.

Honest Guidance

We tell you what you actually need — not what generates the most revenue. If a service isn't right for you, we'll say so. Our goal is your wellbeing, not a bigger invoice.

Prevention Over Reaction

We believe the best care happens before a crisis. Our RN-led model is built around early intervention, proactive monitoring, and education — keeping you home and out of the hospital.

Private Pay Service

Hope Is At Home is a private pay service. We do not bill Medicare, Medicaid, or insurance directly — our services are paid for out of pocket by clients and their families.

Have a Long-Term Care Insurance Policy?

If you or your loved one holds a long-term care insurance policy, you may have benefits available to help offset the cost of home care. While we are not a billing agent for insurance companies, our RN can help you understand what your policy may cover, what documentation is typically required, and how to work with your insurer to access your benefits. We're happy to provide the clinical documentation and support you need to submit a claim.

Not sure whether your policy applies? Bring it to your initial consultation and we'll review it together.

Why Families Choose Hope Is At Home

The Owner Evaluates You Personally

Your care journey starts with a face-to-face visit from our founder and RN — not a call center intake or a sales appointment. She listens, assesses, and builds your plan herself with yours and your families' input.

Broad Clinical Nursing Expertise

From medication review and chronic disease monitoring to post-hospital coordination and home safety assessment, our RN brings deep clinical knowledge directly into your home — helping reduce the need for facility-based care.

Continuous RN Oversight

Unlike many referral agencies, we maintain active nursing oversight throughout your care — not just at intake. Your nurse monitors your condition, adjusts your plan, and stays engaged at every stage.

Deep Roots in Buffalo

We're a local, independent service — not a franchise. We know Western New York, we know its families, and we're invested in the health of our community.

Trusted Referrals When You Need More

When needs evolve beyond what home care can provide, we connect families with trusted partners — including Oasis Senior Advisors and Caring Transitions — so the next step is never something you navigate alone.

Preventing Hospitalizations

Our proactive approach has helped families avoid unnecessary ER visits and hospital stays. Early intervention and skilled nursing monitoring are our specialties — and your peace of mind is our measure of success.
