Care Services Designed Around You

From skilled nursing consultation to everyday support, every service is tailored to your needs — and delivered with the dignity and compassion you deserve.

You Choose What You Need

At Hope Is At Home your evaluation, recommendations, referrals and care plan are built around your actual needs and wants — not a one-size-fits-all program. Choose individual services, combine them, or adjust as your needs change. We're here to support your independence, not replace it.  Hope Is At Home will provide your recommendations and referrals to help you stay safe, healthy and independent for as long as you can while keeping you in the drivers seat of your care and your health.

01

Family-Directed Care

You stay in control.

We believe families know their loved ones best. Our family-directed care model puts you at the center of every decision — you choose the caregivers, set the schedule, and remain in the driver's seat from start to finish. Elizabeth and Richard's combined decades of experience in hospital, rehabilitation, and community healthcare settings have cultivated a trusted network of experienced, pre-vetted private caregivers — including CHHAs, LPNs, and RNs — who can be referred to your family based on your specific needs and preferences. Hope Is At Home provides referrals only; caregivers are privately hired and employed directly by the family, and work under the family's direction within their individual professional scope of practice.

  • Family directs and privately employs all caregivers — you remain in full control
  • Referrals drawn from Elizabeth & Richard's trusted network of pre-vetted CHHAs, LPNs, and RNs
  • RN care plan developed to guide your family and caregivers throughout the care journey
  • Hope Is At Home provides referrals— not a licensed HCSA or CHHA under NYS DOH
02

In-Home RN Consultation & Assessment

Clinical expertise at your door.

Our registered nurses bring hospital-level clinical knowledge directly into your home in the form of professional consultation and assessment. We conduct thorough in-home evaluations, develop personalized care plans, coordinate with your physicians and care team, and provide ongoing monitoring and guidance — helping you and your family make informed decisions about your care. All nursing services requiring licensure under a Home Care Services Agency are referred to appropriately licensed providers.

  • Comprehensive in-home RN assessment and care planning
  • Coordination with physicians and licensed care providers
  • Ongoing monitoring, guidance, and family communication
03

Post-Hospital & Rehab Recovery Support

Bridging the gap between discharge and full recovery.

The weeks following a hospital stay or rehabilitation discharge are among the most critical — and most vulnerable — in a person's recovery. Our RN provides focused consultation and coordination to support a safe transition home: reviewing discharge instructions, monitoring for warning signs, coordinating follow-up care, and keeping your family informed every step of the way. When skilled nursing or therapy services are needed, we coordinate referrals to appropriately licensed home care agencies.

  • Discharge review and transition planning with your RN
  • Monitoring for complications and early warning signs
  • Referrals to licensed agencies when skilled services are needed
04

Medication Review & Education

Reducing risk, improving outcomes.

Medication errors are one of the leading causes of preventable hospitalizations among seniors. Our RN conducts thorough medication reviews, identifies potential interactions or concerns, and provides education to help clients and families understand their medications — including proper timing, dosing, and what to watch for. When medication administration support is needed, we coordinate referrals to appropriately licensed home care providers.

  • Comprehensive medication review and reconciliation by RN
  • Identification of potential interactions and concerns
  • Patient and family education on medications and adherence
05

Chronic Disease Monitoring & Education

Staying ahead of complications.

Living with a chronic condition doesn't have to mean frequent hospitalizations or a declining quality of life. Our RN works proactively to monitor your condition, recognize early warning signs, coordinate with your physician, and educate you and your family on how to manage your health confidently at home — reducing the risk of preventable complications and emergency visits.

  • Proactive monitoring of chronic conditions
  • Early intervention and physician coordination
  • Patient and family education and empowerment
06

Home Safety Assessment

Making your home as safe as possible.

A safe home environment is the foundation of successful home care. Our RN conducts a thorough room-by-room safety evaluation, identifying fall hazards, mobility barriers, and environmental risks. We then provide a detailed written report with prioritized recommendations — and connect you with trusted local vendors to implement them, from grab bars and ramps to raised toilet seats and non-slip surfaces. Our goal is simple: to help you stay home safely, for as long as possible.

  • Comprehensive RN-led room-by-room safety evaluation
  • Written recommendations for modifications and adaptive equipment
  • Referrals to trusted local vendors for installation and equipment
07

Housekeeping Coordination

A safe, clean home supports better health.

A tidy, organized home isn't just comfortable — it's safer. Clutter, dust, and disorganization can contribute to falls, respiratory issues, and stress. We help coordinate light housekeeping support that keeps the home environment safe, clean, and conducive to healing and independence.

  • Light housekeeping and tidying support
  • Fall hazard reduction through organization
  • Coordination with your existing care team
08

Doctor Appointment Coordination

Nothing falls through the cracks.

Keeping up with multiple providers, follow-up appointments, and medical instructions can be overwhelming. We help schedule appointments, prepare questions, accompany or coordinate transportation, and follow up on instructions — so your medical care stays organized and nothing important gets missed.

  • Scheduling and follow-up coordination
  • Preparation for appointments and procedures
  • Communication between providers and family
09

Family & Caregiver Training

Confidence and safety for the people who care most.

Sometimes what a family needs most isn't a caregiver — it's the knowledge and confidence to safely care for their loved one themselves. Our RNs come directly to your home to provide hands-on training for family members and privately hired caregivers, covering the specific skills and equipment relevant to your loved one's needs. From operating a Hoyer lift or sit-to-stand equipment, to safe showering and bathing techniques, proper positioning, transfers, and medication administration — we make sure everyone involved in your loved one's care feels prepared, capable, and safe. Caring for someone you love is one of the most meaningful things you can do. We're here to make sure you can do it without risking your own health or your loved one's safety.

  • Hands-on RN training in your home, tailored to your loved one's specific needs
  • Equipment operation: Hoyer lifts, sit-to-stand devices, wheelchairs, and more
  • Safe transfer, positioning, bathing, and personal care techniques
  • Medication administration guidance and caregiver confidence building

Hope Is At Home is a private-pay RN consultation and care coordination service. We are not a licensed Home Care Services Agency (HCSA) or Certified Home Health Agency (CHHA) under the NYS Department of Health. Skilled nursing procedures and personal care services requiring HCSA or CHHA licensure are referred to appropriately licensed providers. Through our Family-Directed Care model, we provide referrals to pre-vetted, experienced private caregivers — including certified home health aides (CHHAs) and licensed practical nurses (LPNs) and registered nurses (RNs) — who are matched to your family's specific needs and work within their professional scope of practice.  For families who choose to utilize the Family-Directed Care model, Hope Is At Home will develop a comprehensive RN care plan to guide the family and their caregivers throughout the care journey.
